Adoption Agreement

This Agreement between ("Adopter") and Stray Cat Alliance, (including Foster Guardians, Board members & Volunteers) (herein referred to as "SCA") dated as of , covers the terms and conditions of Adoption of:

Adopter and SCA agree to adoption of Cat subject to the Terms and Conditions outlined below. The Adoption Questionnaire, this Agreement and the Terms and Conditions herein shall remain in effect for the remainder of the life of the Cat.
 
Terms and Conditions:
 
  1. Adopter agrees that adoption of the Cat is a commitment for the life of the Cat, which can be 15 to 20 years. However, in the event Adopter cannot keep the Cat, Adopter agrees to return Cat to no place or person other than SCA. Cat may not be sold, given away, turned into an animal shelter, research facility or placed in another permanent home or organization without written permission from SCA.
  2. Cat is being adopted as a pet ONLY and will never be used for any other purpose, including but not limited to, laboratory experiment and research, rodent control, etc. Cat will be loved, respected and treated with kindness. Adopter will ensure that the Cat is never subjected to cruel or inhumane treatment.
  3. Adopter has the owner or landlord's permission to keep the Cat.
  4. Adopter agrees to read handouts from SCA about introducing Cat to new home.
  5. Adopter will provide Cat with premium-grade food and fresh, filtered water on a daily basis and will provide cat with a hygienic, daily-cleaned litter box and an indoor area from which escape is impossible.
  6. Adopter will keep Cat INDOORS AT ALL TIMES and will not permit Cat to go outside.
  7. Adopter will provide appropriate scratching posts or other surfaces for Cat to scratch. Adopter will NOT DECLAW Cat under any circumstances.
  8. Adopter agrees to keep the Cat free from fleas, ticks, mites, and worms and to apply or administer ONLY veterinarian approved treatments (Advantage, Frontline, etc.).
  9. Adopter acknowledges that there may be a period of adjustment for the Cat and Adopter agrees to follow the advice given to them by SCA regarding the Cat's adjustment period.
  10. Adopter will arrange for proper veterinary treatment for the Cat. Adopter will take Cat to a vet for annual examinations, or if the Cat becomes ill or injured. Adopter will be solely responsible for all veterinary bills and other expenses related to the Cat.
  11. Adopter will never consider euthanasia as alternative to medical treatment and will obtain a diagnosis including blood tests by a licensed veterinarian. Adopter will make all veterinarians medical records for Cat available to SCA upon request.
  12. Adopter may return the cat to SCA for any reason within 72 hours of SCA's home delivery for a full refund. After that time, if for any reason Adopter is no longer able to care for the Cat, Adopter agrees to notify and return the Cat to SCA, but Adopter's fee will not be refunded.
  13. Adopter understands and agrees that SCA cannot and does not guarantee or warrant the health or future behavior of Cat. Accordingly, SCA, its officers, volunteers, veterinarians and foster parents of Cat will not be responsible for, and Adopter releases all of them from any claim or liability resulting from, any pre-existing or future illnesses or medical conditions of Cat or any damage, loss or injury that Cat may cause.
  14. Adopter assumes full financial responsibility for the care and maintenance of the Cat as of the date of this agreement.
  15. Adopter agrees to carry emergency contact information for the care of Cat. Adopter agrees to write out instructions for the care of Cat in the event that Adopter predeceases Cat.
  16. If Adopter returns cat for any reason, Adopter will not assert any demand against SCA for any claims, losses, costs, damages, liabilities and expenses that may have been incurred.
  17. Adopter will notify SCA immediately within 24 hours if Cat is lost or stolen and will work with SCA to recover him/her. Adopter will keep a safe (breakaway or stretch) collar and current ID tag on Cat at all times.
  18. Adopter will notify SCA of change of residence address or telephone number within 30 days of change.
  19. Adopter acknowledges that SCA may contact Adopter and make follow-up visits to ensure the well-being of Cat. Adopter will provide truthful information about his/her condition and if Adopter violates any of the terms of this agreement, SCA may repossess the Cat from the Adopter.
  20. Answers to Adoption Questionnaire dated are truthful and considered part of the Agreement.
  21. Failure to comply with these terms and conditions may result in the immediate forfeiture of the Cat and Adopter will surrender Cat upon request of SCA and will not pursue any legal action against SCA, Foster Guardians, Board members, veterinarians or Volunteers.
  22. Failure to abide by the above terms is a material breach of this Agreement. Adopter acknowledges that damages in the event of such breach would be extremely difficult to determine and therefore agrees to pay SCA the sum of $2,000.00 (Two Thousand Dollars) as liquidated damages for any such breach and authorize SCA to reclaim the Cat as stated in Paragraph 19. Adopter further acknowledges that SCA may make follow-up visits and or calls to ascertain the above conditions are being satisfied.
  23. Adopter will notify SCA by email or phone within 24 hours if Cat becomes critically ill and/or dies from any cause.
  24. Adopter understands that the Avid microchip registration will remain in the name of Stray Cat Alliance.

By clicking "Submit Adoption Agreement," I, the Adopter, agree to the Terms and Conditions outlined in the Agreement, and that this Agreement and the Terms and Conditions herein shall remain in effect for the remainder of the life of the Cat.
